WHY: Covenant Cares is how Covenant caregivers help fund needed equipment and projects through the Employee Fund Distribution Board. Covenant Cares funds are caregiver-donated funds that are administered and distributed by caregivers.
WHAT: 2018-4th Quarter Covenant Cares Application solicitation.
WHEN: Deadline for submitting your Funding Requests/Application for Equipment or Programs is January 11.
WHERE: The Share Drive in the PUBLIC- FD under Covenant Cares is where you can find your forms.
HOW TO APPLY:
Fill out the request form (it’s important you answer every question on the request form – note the changes to the funding request; question 7 is very important).
Attach a valid COVENANT vendor quote (quote needs to hold for at least 30 days after the January 11 deadline, if possible) for the equipment you are requesting
Obtain all required signatures.
Call or email Lerica Eaton with any questions concerning the funding application and Finance for questions regarding the capital process.
IMPORTANT THINGS TO REMEMBER:
- Don’t forget to include any necessary shipping and handling costs
- Covenant Cares will only pay for what is included on the quote and awarded.
- Internet quotes cannot be accepted…. (this has to be a valid quote from a Covenant vendor – we will not accept internet quotes).
- Internet companies are not valid vendors!
- We cannot accept applications if the quote is not from a Valid Covenant Vendor
- If you have received funding for equipment in the past and you have not submitted your End of Project Report you cannot apply for new funding until that is turned in.
(Please pay close attention to rules on vendor quotes in the “How” section) - Corporate Policy Change:
“Corporate policy has recently changed around use of funds for furniture and renovations. This change states that any renovation or furniture request under $100,000 must be operationalized and cannot be paid for from capital dollars. This puts an enormous strain on operational dollars as well as Covenant Cares funds; therefore we are proposing a potential solution. If you need renovation to your area, or furniture for your area I would request those items are sent to me, so that I can bundle as many as possible to see if I can make multiple projects hit the $100,000 minimum requirement and therefore use Covenant Cares funds for patient needs and to make sure we hit our operational budgets
